Location plays a significant role in your healthcare options. In 2024, Devoted Health offers Medicare Advantage plans in 13 states, each with unique plans tailored to specific coverage needs. Devoted is expanding from 13 states in 2024 to 20 states in 2025. The Devoted Health footprint will cover 36.5% of Medicare Advantage enrollees nationally.

A Chronic Special Needs Plan (C-SNP) provides benefits and services to people with specific conditions and or certain health care needs.  Special Needs Plans include care that is beneficial with the member needs. These tailor their benefits, provider choices, and list of covered drugs to best meet the specific needs of the groups they serve.


SNPs are either HMO or PPO plan types. Devoted provides two plan offerings that are C-SNP and both are Health Maintenance Organization's (HMO). C-SNP's cover the same Medicare Part A and Part B benefits that all Medicare Advantage Plans cover. However, SNPs might also cover extra services for the special groups they serve.


In order to be eligible for Devoted C-SNP plans, a member must have diabetes or a qualifying cardiovascular condition.


There are two C-SNP plans offered by Devoted. A member may choose to pay a premium to receive a higher monthly amount preloaded to their Home and Food Card. The Home and Food Card is used to purchase of groceries, utilities, mortgage, or rent. The other C-SNP does not require premium payment, but the Home and Food Card amount is less.

Beginning with Devoted Core Arizona and Extra HMO are plans that provided well-rounded benefits with the extras. Prime HMO has a premium for those members who would like to pay more in exchange for the richest supplemental benefits. Prime HMO is available to members with qualifying conditions that will enable them to receive a Home and Food Card. Devoted offers plans with a Part B premium reduction. One plan is designed as a MAPD and the other is designed as an MA. MAPD is a Medicare Advantage plan with a Part D prescription drug plan included. The MA is a Medicare Advantage plan that does not include a Part D prescription drug plan. The MA plan is available to anyone, but was especially designed for veterans who receive their prescriptions from the Veterans Administration. A member can have up to $145 per month added to their Social Security Check each month. The giveback benefits is provided in exchange for other medical benefits.


Devoted Choice Arizona and more

The PPO plans offered by Devoted include plans such as Choice Arizona PPO and additional Part B giveback plans. The Choice PPO plan is for members who value network flexibility, well-rounded benefits, and extra supplemental benefits. Network flexibility is provided by co-payments for service outside the established network versus the industry standard co-insurance requirements. This plan requires low daily co-payments for the first seven days of hospitalization. In-Network co-payments are $0 for all the following plan options:
