Submit Medicare Supplement Application
Submit an application if:
1. You are within 6 months of your Part B effective date and are 65 years or older, or
2. you currently have Medicare Supplement Insurance. (you will have to be approved through underwriting if you already have a Medicare supplement.
3. Do not submit an application if you already have a Medicare Supplement policy and one or more of these conditions.
Submit Advantage Plan Application
Choose from our Medicare Advantage Plan Carriers if one of the following applies to you:
1. You have your Medicare Card in your possession.
Either #2 or #3 is applicable:
2. It is the 3 months prior to your birthday month, the month of your birthday, or the three months after your birthday month.
3. It is between October 15 - December 7 also known as the Annual Enrollment Period.
